Why this applies to Ticagrelor

Ticagrelor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the thing about Brilinta (ticagrelor): your liver uses an enzyme called CYP3A4 to break it down. Ceylon Leadwort contains a compound called plumbagin that, in test-tube studies, slows that same enzyme down. If that effect also happened in people, your body might clear ticagrelor more slowly, letting its levels creep up. Ticagrelor is an active blood thinner already (not a prodrug), so higher levels could mean a greater chance of bruising or bleeding, things like nosebleeds, bleeding gums, or blood that's slow to stop.

I want to be honest though: this is based only on lab research, not real people. It's a theoretical concern, not a proven one. Still, since bleeding risk is serious, please run it by your pharmacist or doctor before combining them.

Ticagrelor also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the thing to keep in mind: ticagrelor (Brilinta) is already a powerful antiplatelet medicine that keeps your platelets from clumping together to form clots. Ceylon Leadwort may do something similar. In animal studies, its root extract reduced platelet stickiness and made bleeding last longer. Stack that on top of ticagrelor and you could, in theory, tip the balance toward too much bleeding.

What might that look like? Easy bruising, nosebleeds, bleeding gums, or blood that takes longer to stop from a small cut. I'll be honest, though: this concern comes only from lab and animal research, not from people, so it's more caution than proven danger. Still, with a clot-preventing drug this important, please loop in your pharmacist or doctor before adding it.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. About Ticagrelor

What Is Ticagrelor Used For?

Brand name: Brilinta

Ticagrelor tablets are used to lower the risk of cardiovascular death, heart attack (myocardial infarction), and stroke in adults who have had acute coronary syndrome (ACS) — a group of conditions including unstable angina and heart attack — or who have a history of heart attack. It also reduces the risk of stent thrombosis (a blood clot forming in a coronary stent) in people who have been stented for ACS treatment.

Ticagrelor tablets are also used in people with coronary artery disease (CAD) who are at high risk for a first heart attack or stroke, particularly those with type 2 diabetes. Additionally, Brilinta is approved to reduce the risk of stroke in adults with a mild acute ischemic stroke or a high-risk transient ischemic attack (TIA, or "mini-stroke").
