Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's the situation with Rexulti (brexpiprazole) and fever bark. Your body relies heavily on a liver enzyme called CYP2D6 to clear brexpiprazole out of your system. Fever bark contains a yohimbine-like compound that may slow that enzyme down, at least in lab tests. If the enzyme works more slowly, brexpiprazole can build up to higher than expected levels.

What might that look like in real life? More of this drug's typical side effects, things like drowsiness, restlessness, dizziness, weight gain, or feeling unusually sluggish. I'll be honest though: this concern comes only from test-tube data, so it's theoretical and the real effect of fever bark in people isn't known. Still, given fever bark's own risky alkaloids, I'd run this by your pharmacist or doctor before combining them.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
