Codeine Phosphate, Pseudoephedrine falls under STIMULANT DRUGS — the group Hordenine interacts with.

Hordenine is structurally similar to N-methyltyramine and synephrine, constituents in bitter orange known to have stimulant properties. Theoretically, taking hordenine with drugs with stimulant properties might increase the risk of hypertension and other adverse cardiovascular effects.
Some of these drugs include amphetamine, caffeine, methylphenidate, pseudoephedrine, and many others.

The piece to focus on here is the pseudoephedrine in this combo product. Pseudoephedrine is itself a stimulant decongestant, and hordenine is marketed as a mild stimulant too. Stacking two stimulant-type substances can add up, so the theoretical concern is a bit more cardiovascular load: higher blood pressure, a faster or pounding heartbeat, jitteriness, or trouble sleeping. The codeine portion isn't really the issue with hordenine.

I'll be honest that this is a theoretical interaction based on how these compounds look and behave, not on solid human studies, so don't panic. Still, since hordenine's safety isn't well established, I'd play it safe, especially if you have any heart concerns or high blood pressure. Run it by your pharmacist or doctor before combining them.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Codeine Phosphate, Pseudoephedrine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Hordenine weakly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, hordenine might increase the levels of CYP2D6 substrates.
Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the twist with this combo. Codeine is actually a prodrug, meaning it doesn't do much on its own. Your body uses an enzyme called CYP2D6 to convert codeine into morphine, which is what gives you the pain relief and cough suppression. Hordenine may weakly block that same enzyme. So instead of building up codeine's effect, slowing CYP2D6 could mean less morphine is made, and you might get weaker pain or cough control.

The good news: this is only suggested by lab (test tube) studies, the inhibition is weak, and it's rated minor. The pseudoephedrine part isn't really affected here. I wouldn't lose sleep over it, but if you take both, mention it to your pharmacist or doctor.
