Acetaminophen, Butalbital, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Lentinan interacts with.

Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

The piece of this combo that matters here is the caffeine. Your body clears caffeine mainly through a liver enzyme called CYP1A2. Lab and animal studies hint that lentinan might speed that enzyme up. If that happened in people, caffeine could be broken down a little faster, so you might notice it wearing off sooner or feeling slightly weaker. The acetaminophen and butalbital parts aren't really driven by this pathway, so the headache relief itself shouldn't change much.

Honestly, this is very early, theory-level evidence and it has never been shown in actual humans. For most people it's unlikely to cause a real problem. Still, if you plan to use a lentinan supplement regularly, it's worth a quick chat with your pharmacist or doctor first.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for your headache combo (Tecnal, Esgic, and similar). Lab studies hint that lentinan might rev up a liver enzyme called CYP3A4, which helps break down the butalbital portion of this medicine. If that enzyme works faster, your body could clear the butalbital a bit quicker, which in theory might make the headache relief feel weaker or wear off sooner. The caffeine and acetaminophen pieces are handled mostly by other pathways, so they're less of a concern here.

The important thing: this is only test-tube and animal research, never actually seen in people, so I wouldn't lose sleep over it. Still, if you start lentinan and notice your headaches aren't easing like usual, check in with your pharmacist or doctor before changing anything.

What Is Acetaminophen, Butalbital, and Caffeine Used For?

Brand names: Tecnal, Esgic Plus, Fiogesic, Triad, Repan, Esgic, Fioricet

This three-ingredient combination — butalbital, acetaminophen, and caffeine — is used to relieve the symptoms of tension headache (also called muscle contraction headache). All supported products, including tablets, capsules, and oral solution, share this same approved use.

It is worth knowing that evidence for using this combination to treat repeated, frequent tension headaches over time is limited. Because butalbital can be habit-forming, extended or repeated use is not recommended.
