Acetaminophen, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Lentinan interacts with.

Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Here's the honest picture: this is a combo cold product, with three active ingredients. The interaction on file is about lentinan possibly speeding up an enzyme called CYP1A2. In theory, if a drug leans on that enzyme, lentinan could clear it a little faster and make it slightly weaker. But this is based only on lab and animal work, and it hasn't actually been seen in people, so I wouldn't lose sleep over it.

The good news for this particular product is that acetaminophen, dextromethorphan, and pseudoephedrine don't depend heavily on CYP1A2, so a big change in how your cold medicine works is unlikely. None of these are prodrugs that need that enzyme to switch on. Still, if you plan to use lentinan regularly, run it by your pharmacist or doctor first.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Out of the three ingredients in these cold and flu products, the one that matters here is dextromethorphan, the cough suppressant. Your liver uses an enzyme called CYP2D6 to break it down. Lab research hints that lentinan might speed that enzyme up a bit. If that happened in real life, your body could clear dextromethorphan a little faster, which might make it work less well for calming a cough.

I want to be straight with you though: this is based only on test-tube and animal studies, and it has never been reported in actual people. So I would not lose sleep over it. The acetaminophen and pseudoephedrine pieces aren't really affected. Still, it's always worth a quick check with your pharmacist before pairing lentinan with any medicine.

Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist with this combo cold and flu product. Lab research hints that lentinan might rev up a liver enzyme called CYP3A4. One of the three ingredients here, dextromethorphan (the cough suppressant), is partly handled by that enzyme. If lentinan speeds up its breakdown, you could clear it a little faster, which might make the cough relief feel weaker. The acetaminophen and pseudoephedrine pieces aren't really affected by this pathway.
