Acetaminophen, Hydrocodone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Lentinan interacts with.

Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Here's the honest picture: this concern comes from lab evidence that lentinan might rev up an enzyme called CYP1A2. The thing is, neither the hydrocodone nor the acetaminophen in your Norco or Vicodin really leans on CYP1A2 to be processed. Hydrocodone is handled mostly by CYP2D6 and CYP3A4, and acetaminophen is cleared by other pathways. So this particular enzyme effect isn't a great fit for this combo.

On top of that, this is only test-tube and animal data, with nothing reported in actual people. I wouldn't lose sleep over it. Still, since these are products you're taking for pain, it's always smart to run any new supplement past your pharmacist or doctor first.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the wrinkle with Vicodin or Norco: the hydrocodone part is actually a prodrug. Your liver uses an enzyme called CYP2D6 to convert it into hydromorphone, which is the piece that does most of the pain relieving. So anything that speeds up CYP2D6 could, in theory, turn hydrocodone into its active form faster.

Lab research hints that lentinan might rev up CYP2D6. If that happened in real life, you could get a bit more opioid effect, meaning stronger drowsiness, constipation, nausea, or slowed breathing. But honestly, this is only test-tube and animal data and has never been reported in people, so I wouldn't lose sleep over it. The acetaminophen side isn't affected. Just give your pharmacist a heads-up before combining them.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for your Vicodin or Norco (the hydrocodone with acetaminophen combo). Hydrocodone is actually a prodrug of sorts. Your liver uses an enzyme called CYP3A4 to break it down, and lab studies hint that lentinan might speed that enzyme up. If hydrocodone clears faster, you could get less pain relief than expected.

Now, the important part: this comes from test tube and animal research only, and it has never actually been seen in real people. So I wouldn't lose sleep over it. The acetaminophen side isn't really affected here. If you notice your pain medicine working less well after starting lentinan, just give your pharmacist or doctor a quick call before changing anything.
