Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist for Rexulti (brexpiprazole): your body relies partly on an enzyme called CYP2D6 to break this medicine down. Lab research hints that lentinan might rev up that enzyme a bit. If that happened in real life, your body could clear brexpiprazole a little faster, which might mean lower levels and a weaker effect. For something treating schizophrenia or depression, that could look like symptoms creeping back, more mood changes, or less benefit from your dose.

I want to be honest, though: this is only seen in test-tube and animal studies, never confirmed in people, so it's more of a theoretical concern than a proven problem. Don't change anything on your own. Just give your pharmacist or doctor a heads-up before adding lentinan so they can keep an eye on how you're doing.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist: your body uses an enzyme called CYP3A4 to help break down brexpiprazole (Rexulti). Lab studies hint that lentinan may speed that enzyme up a bit. If that happened in people, your body could clear brexpiprazole faster, which might leave a little less of it working. In practice that could mean your symptoms (whether that's schizophrenia or depression you're managing) aren't as well controlled as before.

I want to be honest about the evidence here: this is only seen in test tube and animal research, and it has not been reported in actual patients. So I wouldn't lose sleep over it. Still, if you're considering lentinan, run it past your pharmacist or doctor first, and let them know if your mood or symptoms seem to slip. Don't change your Rexulti on your own.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
