Buproprion, Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Auvelity is a clever two-part pill. The bupropion piece is there partly to block the CYP2D6 enzyme, which keeps the dextromethorphan piece from being broken down too fast so it can stay in your system and do its job in the brain. Lab research hints that lentinan might nudge CYP2D6 to work a bit faster. If that happened, it could in theory lower the dextromethorphan levels and make Auvelity feel less effective for your mood.

Here's the reassuring part: this is only test-tube and animal data, and it has never actually been seen in people. So I wouldn't panic. Still, if you notice your depression symptoms creeping back while using lentinan, mention it to your pharmacist or doctor rather than changing anything on your own.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Auvelity combines dextromethorphan and bupropion, and it works in a clever way. The bupropion isn't just an antidepressant here, it actually slows down the enzyme that clears dextromethorphan, letting the dextromethorphan build up enough to act on your brain. The CYP3A4 pathway also plays a role in breaking down these ingredients.

The lab research suggests lentinan might speed up CYP3A4. In theory, that could clear these drugs a bit faster and make Auvelity work less well, meaning your depression symptoms might not be as well controlled. I want to be honest though: this has only been seen in test tubes and animals, never in real people, so it's a weak signal. Still, it's worth mentioning to your pharmacist or doctor before adding lentinan.
