Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

This combo cough and cold product contains two active pieces. The dextromethorphan (the cough suppressant) leans heavily on a liver enzyme called CYP2D6 to be broken down. Lab studies hint that lentinan might speed that enzyme up, which could clear dextromethorphan a bit faster and make it work less well, so your cough might not be as controlled. The chlorpheniramine (the antihistamine) is also partly handled by this pathway.

Here is the reassuring part: this is only seen in test-tube and animal research, never reported in real people, so it is mostly theoretical. Neither drug is a prodrug here, so the likely direction is slightly weaker effect, not toxic buildup. Still, if you regularly take lentinan, mention it to your pharmacist or doctor.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

This combo cold medicine pairs chlorpheniramine (an antihistamine) with dextromethorphan (a cough suppressant). Lentinan has been seen in lab and animal studies to nudge up the activity of an enzyme called CYP3A4, which helps clear dextromethorphan from your body. In theory, speeding up that enzyme could lower drug levels a little, meaning your cough relief might not last as long or feel as strong.

Honestly, though, this has never been reported in real people, and it comes only from test-tube and animal research, so I wouldn't lose sleep over it. Neither of these is a prodrug, and this is a short-term cold remedy. If you notice it just isn't working, mention it to your pharmacist before changing anything.
