Why this applies to Ibuprofen, Hydrocodone

Ibuprofen, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the wrinkle with this combo product. The hydrocodone half is actually a prodrug: your body uses the enzyme CYP2D6 to turn it into hydromorphone, the piece that does most of the pain relief. Lab studies hint that lentinan might speed up CYP2D6. If that happened in a real person, you could end up making a bit more of that active painkiller, which in theory could mean more drowsiness, dizziness, nausea, or constipation.

The honest part: this is only test-tube and animal data, and it has never been seen in people. So I wouldn't lose sleep over it. The ibuprofen side isn't really affected here. Still, before adding any lentinan supplement, run it by your pharmacist or doctor so they know your full picture.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

This combination pill pairs ibuprofen (an anti-inflammatory pain reliever) with hydrocodone (an opioid). The hydrocodone part is the piece that matters here. Hydrocodone is actually a prodrug in part, and the liver enzyme CYP3A4 helps process it. Lab and animal research hints that lentinan may slightly speed up CYP3A4. If that happened in people, your body could clear the hydrocodone a bit faster, which might mean less pain relief than expected.

The good news: this has only been seen in test-tube and animal studies, never reported in real patients, so I wouldn't lose sleep over it. Still, if you notice your pain not being controlled as well, mention it to your pharmacist or doctor before changing anything.
