Ivacaftor, Lumacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for Orkambi (lumacaftor/ivacaftor). The ivacaftor part is cleared from your body largely by a liver enzyme called CYP3A4. Lab and animal research hints that lentinan might rev that enzyme up a bit. If that happened in real life, your body could break ivacaftor down faster, leaving less of it around and possibly making it work less well. For cystic fibrosis, that could mean a little less help with mucus, breathing, and digestion over time.

I want to be honest about the evidence though: this is only test-tube and animal data, and it has never been confirmed in people. So I wouldn't panic. Still, because keeping a steady level of this medicine matters, please run any lentinan supplement past your CF team or pharmacist first.

What Is Lumacaftor and Ivacaftor Used For?

Brand name: Orkambi

Lumacaftor and ivacaftor (Orkambi) treats cystic fibrosis (CF) in people aged 1 year and older who are homozygous for the F508del mutation in the CFTR gene. Homozygous means you inherited the mutation from both parents.

If your genetic makeup is unknown, an FDA-cleared CF mutation test is used to check for the mutation on both copies of the gene. Orkambi has not been shown to be safe or effective in people with CF who do not have two copies of F508del.
