Mexiletine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Lentinan interacts with.

Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Here's the basic idea with mexiletine and lentinan. Your liver uses an enzyme called CYP1A2 to clear mexiletine out of your body. Lab and animal studies hint that lentinan may speed up that enzyme. If that happened in people, your body could remove mexiletine a little faster, which might lower its level in your blood and make it work less well at keeping your heart rhythm steady. Mexiletine isn't a prodrug, so faster clearance means less effect, not more.

I want to be honest about the strength here: this is based only on test-tube and animal data, and it hasn't actually been seen in humans. So please don't panic. Since mexiletine controls a serious heart rhythm problem, even small shifts matter, so check with your pharmacist or doctor before adding lentinan.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist for mexiletine: your body relies heavily on a liver enzyme called CYP2D6 to clear this medication out. Lab studies hint that lentinan might rev that enzyme up a bit. If it does, your system could break down mexiletine faster, leaving less of it around to keep your heart rhythm steady. Mexiletine is an active drug (not a prodrug), so faster clearance points toward a weaker effect, not a stronger one.

I want to be honest about the evidence here, though. This is only test-tube and animal research, and it has never actually been seen in people taking lentinan. So I wouldn't lose sleep over it. Still, since mexiletine controls a serious heart rhythm, please loop in your doctor or pharmacist before adding lentinan so they can watch for any return of palpitations.

About Mexiletine

What Is Mexiletine Used For?

Brand name: Mexitil

Mexiletine Hydrochloride capsules are indicated for the treatment of documented, life-threatening ventricular arrhythmias — for example, sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers). Because mexiletine itself can trigger arrhythmias, it is not recommended for minor or symptom-free irregular heartbeats.

Treatment is started in a hospital setting so heart rhythm can be monitored closely. Antiarrhythmic drugs, including mexiletine, have not been shown to improve overall survival in patients with ventricular arrhythmias.
