Why this applies to Selpercatinib

Selpercatinib falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.

Theoretically, concomitant use with hepatotoxic drugs might increase the risk of adverse hepatotoxic effects.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.

Here's the concern with pairing these two: Selpercatinib (Retevmo) is a cancer medicine that can be hard on the liver on its own. Doctors actually check your liver enzymes regularly while you're on it. Ostarine, the SARM you're asking about, has also been linked to liver injury, and some research shows it can push up ALT, one of the blood markers we watch for liver stress.

So the worry isn't a chemical tug-of-war over how the drug is broken down. It's simpler than that: two things that can each stress the liver, stacked together, may add up to a higher chance of trouble. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual fatigue. This is theory-based, not proven, but Ostarine is an unapproved, banned substance with real safety reports. Please loop in your oncologist before combining anything with Retevmo.

Selpercatinib also falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS.

Theoretically, concomitant use of ostarine with CYP3A4 inhibitors could increase the effects and adverse effects of ostarine, although this is unlikely to be clinically significant.
Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's the situation with these two. Selpercatinib (Retevmo) can act as a mild blocker of an enzyme called CYP3A4, and Ostarine is partly cleared by that same enzyme. In theory, slowing down that pathway could let a bit more Ostarine build up, which might mean stronger Ostarine effects and a higher chance of its side effects, including liver strain.

The good news is that this concern is mostly on paper. When researchers paired Ostarine with itraconazole (a much stronger CYP3A4 blocker than Selpercatinib), Ostarine levels barely budged. So a real-world problem here is unlikely. That said, Ostarine is an unapproved SARM tied to liver injury, and you're managing a serious cancer. Please loop in your oncologist or pharmacist before combining anything with Selpercatinib.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
