However, research findings on this matter are conflicting. Here's the connection worth knowing. Puravive contains Panax ginseng, and one of the two medicines in Cabenuva, rilpivirine, can slightly affect your heart's electrical timing (what doctors call the QT interval). Ginseng may add a little to that effect. When the QT interval stretches too far, it can, in rare cases, trigger an abnormal heart rhythm. That might feel like a racing or fluttering heartbeat, dizziness, or fainting.

I want to be honest about the evidence here: it's mixed and fairly weak. Short-term ginseng studies showed a small QT bump, but longer use didn't. For most people this is a low-level concern, but since Cabenuva is a long-acting injection you can't easily undo, it's smart to mention Puravive to your pharmacist or HIV provider before combining them.

Cytochrome P450 3A4 (Cyp3A4) Substrates

Cabotegravir, Rilpivirine also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the thing with Cabenuva (the cabotegravir and rilpivirine combo): the rilpivirine part is partly cleared by a liver enzyme called CYP3A4. Puravive contains Panax ginseng, which in some studies seems to speed up CYP3A4 and in others seems to slow it down. The honest answer is we don't really know which way it would go, and the evidence here is weak and theoretical.

My concern with HIV medicines is keeping the drug level steady. If ginseng nudged rilpivirine levels lower, there's at least a theoretical worry about the virus not being fully controlled over time. This isn't a strong or proven interaction, so don't panic, but please run Puravive past your HIV provider or pharmacist before starting it.

What Is Cabotegravir and Rilpivirine Injections Used For?

Brand name: Cabenuva

CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.

The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
