Ranitidine Bismuth Citrate falls under P-Glycoprotein Substrates — the group Puravive interacts with.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist: Puravive contains quercetin, which lab work suggests can block a gut transporter called P-glycoprotein. Ranitidine bismuth citrate (Tritec) gets moved around partly by that same pump, so in theory quercetin could nudge how much of the medicine gets absorbed. Honestly, the science here is mixed. With some drugs quercetin raised blood levels, with others it lowered them, so we can't say for sure which way this one would go.

The good news is this is rated a minor, theoretical interaction. Ranitidine is a forgiving drug (not a prodrug, and not narrow-margin), so any small shift is unlikely to cause real trouble beyond perhaps slightly more or less heartburn relief. Still, since Puravive isn't well studied, run it by your pharmacist or doctor first.

About Ranitidine

What Is Ranitidine Used For?

Brand name: Tritec

Ranitidine treats a range of stomach acid–related conditions. Prescription-strength ranitidine (capsules, tablets, and oral solution) is used for active duodenal ulcers (ulcers in the small intestine), active benign gastric ulcers (stomach ulcers), GERD (acid reflux disease), erosive esophagitis (acid damage to the esophagus), and pathological hypersecretory conditions — rare disorders where the stomach makes far too much acid, such as Zollinger-Ellison syndrome. It is also used as maintenance therapy to keep duodenal and gastric ulcers from coming back after they have healed.

Over-the-counter products — including Acid Reducer and Acid Reducer 150 tablets — are approved to relieve heartburn associated with acid indigestion and sour stomach, and to prevent that heartburn when taken before eating or drinking foods that trigger it.
