Why this applies to Acalabrutinib

Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist: your body clears acalabrutinib (Calquence) using a liver enzyme called CYP3A4. In a test tube, red clover looked like it might slow that enzyme down a bit. If it really did, acalabrutinib could build up and you might notice more of its usual side effects, like headaches, bruising or bleeding, or a higher chance of infections.

The reassuring part is that this concern is mostly theoretical. When researchers actually tested red clover isoflavones in people using another CYP3A4 drug, they saw no real change. So a meaningful interaction here is unlikely. Still, acalabrutinib is a serious cancer medicine, so please check with your oncologist or pharmacist before adding red clover rather than starting it on your own.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
