Acetaminophen, Doxylamine, Pseudoephedrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version: the worry on paper is that red clover might slow down a liver enzyme called CYP1A2, which could let certain medicines build up. With this combo product (acetaminophen for pain, doxylamine for sleep, and pseudoephedrine for congestion), even if levels nudged up a bit, you might in theory feel a little extra drowsiness from the doxylamine or more jitters from the pseudoephedrine. But honestly, this is very weak.
The lab dish suggested a possible effect, yet an actual human study found red clover did not change how the body handles a CYP1A2 test drug. So this is rated minor and unlikely. I wouldn't lose sleep over it, but if you take red clover regularly, just mention it to your pharmacist.
