Bendamustine Hydrochloride falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the short version on pairing red clover with bendamustine. In a test tube, the isoflavones in red clover can slow down a liver enzyme called CYP1A2, which plays a role in clearing bendamustine. In theory, blocking that enzyme could let a bit more drug linger in your body, which might mean stronger effects like low blood counts, nausea, fatigue, or higher infection risk. But this is a theoretical concern only, based on lab work, and a small human study found red clover didn't actually change how the body handled a CYP1A2 test drug.

So real-world worry here is low. Still, bendamustine is serious chemotherapy, so please don't add red clover without first running it by your oncology team or pharmacist.

What Is Bendamustine Injection Used For?

Brand name: Bendeka

Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.

For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
