Cobimetinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Your body clears cobimetinib (Cotellic) mainly through a liver enzyme called CYP3A4. In a test tube, red clover looked like it might slow that enzyme down a little, which in theory could let cobimetinib build up and raise the odds of side effects like diarrhea, rash, feeling tired, or changes in vision. But this is mostly lab-dish data.

Here's the reassuring part: when researchers actually gave people standardized red clover for two weeks, it did not change levels of a drug that the same enzyme handles. So a real-world problem here looks unlikely. Because cobimetinib is a serious cancer medicine, please still run any supplement past your oncology team before adding it.

About Cobimetinib

What Is Cobimetinib Used For?

Brand name: Cotellic

Cotellic (cobimetinib) is used in two ways. First, it treats adults with unresectable (cannot be surgically removed) or metastatic (spread to other parts of the body) melanoma that carries a BRAF V600E or V600K gene mutation — but only in combination with vemurafenib, another cancer drug. A tumor test must confirm the BRAF mutation before starting this combination.

Second, Cotellic is used on its own as a single agent to treat adults with histiocytic neoplasms, a group of rare cancers involving immune cells called histiocytes.
