Enasidenib falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version on combining red clover with enasidenib (Idhifa). Your leukemia medicine is partly cleared by a liver enzyme called CYP1A2. In a test tube, red clover's plant estrogens can slow that enzyme down, and in theory that could let enasidenib build up a little, which might mean more of its usual side effects like nausea, diarrhea, tiredness, or changes in blood counts.
But here's the reassuring part: this is mostly lab-dish evidence, and a real human study found red clover didn't actually change how the body handled a CYP1A2 test drug. So a meaningful interaction is unlikely. Still, enasidenib is serious cancer therapy, so please run any supplement past your oncology team or pharmacist before starting it.

In vitro evidence suggests that red clover weakly inhibits CYP2C19. This interaction has not been reported in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version on combining red clover with enasidenib (Idhifa). Your body partly clears enasidenib using a liver enzyme called CYP2C19. In a test tube, red clover's plant estrogens appear to slow that enzyme down a little. If that happened in real life, enasidenib could build up slightly, which in theory might mean a bit more of its known side effects, like nausea, diarrhea, or changes in liver and blood test numbers.
That said, this is very weak, lab-only evidence, and it has never actually been reported in people taking these together. I wouldn't lose sleep over it, but since enasidenib is a serious cancer drug, please run any new supplement by your oncology team or pharmacist first.

The thinking goes like this: enasidenib is partly handled by a liver enzyme called CYP2C9, and lab (test-tube) studies hint that red clover's isoflavones might slow that enzyme down a bit. If that happened in the body, enasidenib levels could creep up, which in theory could mean more of its known side effects like nausea, fatigue, low appetite, or changes in your blood counts and bilirubin.
But I want to put your mind at ease. This is theoretical only, based on lab data, and a small human study showed red clover did not actually change how a CYP2C9 drug behaved. The expected risk is minor and unlikely. Still, enasidenib is serious cancer therapy, so loop in your oncology pharmacist or doctor before adding any supplement.

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
