Why this applies to Etonogestrel

Etonogestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the bottom line on combining red clover with your etonogestrel implant (Implanon). The theory is that red clover's plant compounds (isoflavones) can slow down a liver enzyme called CYP1A2, which could in turn raise levels of medicines that enzyme breaks down. Since etonogestrel isn't a prodrug, higher levels would just mean a slightly stronger hormone effect, not a weaker one, so this wouldn't put your birth control at risk.

The good news: this concern comes only from lab dish studies, and an actual human study found red clover capsules had no measurable effect on a CYP1A2 test drug. So this interaction is rated minor and unlikely. Still, if you have a hormone-sensitive condition, run it by your pharmacist or doctor first.

If you're using the etonogestrel implant (Implanon or Nexplanon) for birth control and wondering about red clover, here's the honest picture. In a lab dish, red clover's plant estrogens looked like they might slow down CYP3A4, the same liver enzyme that breaks down etonogestrel. On paper, slowing that enzyme could let a bit more hormone hang around. But when researchers actually tested red clover capsules in people using a CYP3A4 marker drug, they saw no real change.

So this is mostly theoretical, and the human evidence points to little or no effect. I wouldn't expect your implant to work differently, but since your contraception matters, it's worth a quick chat with your pharmacist or doctor before adding red clover, especially given its mild estrogen-like activity.
