Why this applies to Nirmatrelvir, Ritonavir

Nirmatrelvir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version: there's a possible interaction on paper, but it looks pretty minor in real life. Paxlovid already contains ritonavir, which deliberately blocks the CYP3A4 enzyme to keep nirmatrelvir levels up. Red clover's isoflavones might nudge that same enzyme in lab tests, which could in theory raise drug levels a little. In practice, though, a small human study found red clover didn't actually change the levels of a CYP3A4 test drug.

Because Paxlovid is only taken for a short 5-day course, and the evidence here is weak, I wouldn't lose sleep over this combo. Still, since Paxlovid interacts with so many things, please run red clover by your pharmacist or doctor before mixing them.
