Red Clover and Ombitasvir, Paritaprevir Ritonavir

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Ombitasvir, Paritaprevir Ritonavir

Ombitasvir, Paritaprevir Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your Technivie combo (ombitasvir, paritaprevir, and ritonavir). Part of this medicine, paritaprevir, is handled by a liver enzyme called CYP3A4. In a test tube, red clover's isoflavones looked like they might slow that enzyme down a little, which in theory could nudge drug levels up and make side effects like tiredness, nausea, or itching a touch more likely.

The good news: when researchers actually tested red clover in people using a CYP3A4 marker drug, they saw no real change. So this concern is mostly theoretical and rated as minor and unlikely. Still, hepatitis C treatment is important to get right, so I'd run red clover past your pharmacist or doctor before adding it, just to be safe.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Ombitasvir, Paritaprevir Ritonavir page

“Technivie (ombitasvir, paritaprevir, and ritonavir) treats hepatitis C, and because a cure depends on holding steady drug levels for the whole course, this list warrants genuine care. Most of the 154 ingredients on file touch the CYP3A4 enzyme pathway, meaning they can push the drug's blood levels up or down.”

About Red Clover

What is Red Clover?

Red Clover
  • Menopause symptoms (hot flashes)
  • High cholesterol
  • Bone health/osteoporosis
  • Skin and hair concerns
  • Women's health

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.

About Ombitasvir, Paritaprevir, and Ritonavir

What Is Ombitasvir, Paritaprevir, and Ritonavir Used For?

Brand name: Technivie

  • The combination of ombitasvir, paritaprevir, and ritonavir is usually used in combination with ribavirin (Copegus, Rebetol), but sometimes it is used alone to treat a certain type of chronic (long-term) hepatitis C infection (swelling of the liver caused by a virus). Ombitasvir is a hepatitis C virus (HCV) NS5A inhibitor. It works by stopping the virus that causes hepatitis C from spreading inside the body. Paritaprevir is a protease inhibitor. It works by decreasing the amount of HCV in the body. Ritonavir is a protease inhibitor. It helps to increase the amount of paritaprevir in the body so that the medication will have a greater effect. It is not known if ombitasvir, paritaprevir, or ritonavir prevent the spread of hepatitis C to other people.
