Sorafenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version: this interaction is mostly theoretical and very unlikely to cause you trouble. Sorafenib is partly broken down by a liver enzyme called CYP3A4. In a test tube, red clover's isoflavones look like they could slow that enzyme down, which in theory might let sorafenib build up a little and raise the chance of its usual side effects, like fatigue, high blood pressure, diarrhea, or hand-foot skin reactions.
The reassuring part: when researchers actually tested red clover in people using another CYP3A4 drug, it didn't change drug levels at all. So real-world risk appears small. Still, because sorafenib is a cancer medicine where steady levels matter, please check with your oncologist or pharmacist before adding red clover rather than starting it on your own.

In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.

Here's the honest picture for Red Clover and Sorafenib (Nexavar). The concern on file isn't really about how your body processes sorafenib. Instead, red clover has been theorized to thin the blood a little, based on lab studies of its genistein content and its small amount of natural coumarins. Sorafenib on its own can already raise bleeding risk in some people, so stacking something with a possible blood-thinning effect raises an eyebrow.
The reassuring part: human studies haven't actually shown red clover affecting clotting or bleeding times, so this is rated minor and unlikely. Still, because you're on cancer therapy, don't add red clover on your own. Run it by your oncology team or pharmacist first, and watch for easy bruising or unusual bleeding.

What Is Sorafenib Used For?
Brand name: Nexavar
Sorafenib tablets are approved to treat three types of cancer. The first is unresectable hepatocellular carcinoma (HCC) — liver cancer that cannot be removed with surgery. The second is advanced renal cell carcinoma (RCC) — kidney cancer that has spread or progressed. Both the brand Nexavar and generic sorafenib tablets are approved for these two uses.
The third use is for locally recurrent or metastatic, progressive differentiated thyroid carcinoma (DTC) — thyroid cancer that has spread or worsened and no longer responds to radioactive iodine treatment. Nexavar and sorafenib tablets are both approved for this use as well.
