Red Clover and Tasimelteon

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Red Clover interacts with whole categories of medication. Tasimelteon falls into 2 of those categories, so it can interact with Red Clover in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Tasimelteon

Tasimelteon falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist of it: your body uses an enzyme called CYP1A2 to clear tasimelteon (Hetlioz) out of your system. In the lab, red clover looked like it might slow that enzyme down a bit. If that happened in real life, tasimelteon could linger longer and feel stronger, meaning more next-morning grogginess, drowsiness, or odd dreams. Tasimelteon is the active drug itself, not a prodrug, so a slowdown would mean more effect, not less.

The good news is this concern is mostly theoretical. A small human study using a similar CYP1A2 marker showed red clover didn't really change anything. So a meaningful interaction here is unlikely. Still, it's worth a quick word with your pharmacist or doctor before adding red clover, especially if you notice extra sleepiness.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Tasimelteon also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.
Severity Minor Likelihood Unlikely Evidence B

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body relies on a liver enzyme called CYP3A4 to clear tasimelteon from your system. In a test tube, red clover looked like it might slow that enzyme down a bit, which in theory could let tasimelteon hang around a little longer and possibly make its sleepiness effects stronger (think next-morning grogginess or feeling extra sluggish).

But the real-world evidence is reassuring. When researchers gave people a standardized red clover dose and checked another CYP3A4 drug, they saw no meaningful change. So this interaction is rated minor and unlikely. Tasimelteon isn't a prodrug, so we don't expect it to lose effect either. Still, it's worth a quick mention to your pharmacist or doctor before combining them.

About Red Clover

What is Red Clover?

Red Clover
  • Menopause symptoms (hot flashes)
  • High cholesterol
  • Bone health/osteoporosis
  • Skin and hair concerns
  • Women's health

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.
