Why this applies to Treprostinil (IV)

Treprostinil (IV) falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Red Clover interacts with.

Although some laboratory research suggests that red clover may have anticoagulant and antiplatelet activity, clinical research has not shown this effect.
In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.

Here's the gist if you're using treprostinil (Remodulin) for pulmonary hypertension. Treprostinil itself relaxes blood vessels and also makes platelets a little less sticky, so on paper there's a worry that adding red clover, which some lab studies hint may have its own mild blood-thinning effect, could stack on top of that and raise bleeding risk (think easy bruising, nosebleeds, or bleeding that's slow to stop).

In real life, though, this concern hasn't panned out. Human studies in postmenopausal women taking red clover for up to two years showed no measurable change in clotting. So this is a theoretical, low-likelihood interaction. That said, treprostinil is a serious infusion medicine, so please loop in your doctor or pharmacist before adding red clover.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. About Treprostinil Injection

What Is Treprostinil Injection Used For?

Brand name: Remodulin

Treprostinil injection is used to treat pulmonary arterial hypertension (PAH, WHO Group 1) — high blood pressure in the arteries that supply the lungs. It works to reduce the symptoms that make exercise difficult. Clinical studies included patients with moderate-to-severe PAH symptoms (NYHA Class II–IV) due to idiopathic or heritable PAH, PAH related to congenital heart defects, and PAH related to connective tissue diseases.

It is also used for PAH patients who need to transition off epoprostenol — a related medication — to slow the worsening of their condition during that switch. The risks and benefits of the transition should be carefully discussed with a specialist before starting.
