Why this applies to Zolpidem
Zolpidem falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist: your body clears zolpidem using a liver enzyme called CYP3A4. In a test tube, red clover looked like it might slow that enzyme down a little. If that actually happened in people, zolpidem could linger longer and feel stronger, meaning more next-morning grogginess, dizziness, or unsteadiness on your feet.
But here's the reassuring part. When researchers actually tested red clover isoflavones in real people using a similar CYP3A4 medication, it didn't change drug levels at all. So this is mostly a theoretical worry, not something seen in practice. Zolpidem is a true sedative (not a prodrug), so the concern would be too much effect, not too little. Still, since both can make you sleepy, it's worth a quick word with your pharmacist.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. What Is Zolpidem Used For?
Brand names: Ambien CR, Intermezzo, Sublinox
Zolpidem is used short-term for insomnia. Most formulations — including Ambien, oral tablets, and Zolpidem Tartrate Capsules — are approved to help people who have trouble falling asleep at bedtime. Zolpidem Tartrate Capsules are specifically indicated for adults under 65.
The extended-release formulations (including Zolpidem Tartrate Extended-Release Tablets) help with both falling asleep and staying asleep through the night. Sublingual tablets (Edluar, Zolpidem Tartrate Sublingual Tablet, and Zolpidem tartrate sublingual) are taken as needed when you wake up in the middle of the night and cannot get back to sleep — but only if at least 4 hours of sleep time remain before you need to be up.
