Diltiazem, Enalapril falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Your combo pill, Teczem, pairs diltiazem (a calcium channel blocker) with enalapril (an ACE inhibitor) to lower blood pressure. The diltiazem part is handled by a liver enzyme called CYP3A4. In lab dish studies, uva ursi seemed to slow this same enzyme down. If that happened in the body, diltiazem could build up a bit and push its effects too far, possibly causing dizziness, an unusually slow heartbeat, swollen ankles, or blood pressure that drops lower than you want.

Here is the reassuring part: this was only seen in test tubes, never shown in actual people, so it is mostly theoretical. Still, since uva ursi can be hard on the liver, please run it past your pharmacist or doctor before taking it alongside Teczem.

Diltiazem, Enalapril also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist for your combination pill that contains diltiazem and enalapril. Uva ursi has been shown in a lab dish to block a little cellular pump called P-glycoprotein, which helps move certain drugs around the body. Diltiazem is one of those drugs. In theory, if that pump slows down, slightly more diltiazem could hang around, which might add to side effects like dizziness, a slower heartbeat, swollen ankles, or a bigger drop in blood pressure.

I want to keep this in perspective though. This was only seen in a test tube, never in actual people, and the interaction is rated as minor. Enalapril isn't really affected this way. Still, since uva ursi is best for short-term use only, please run it by your pharmacist or doctor before adding it.
