Why this applies to Aliskiren

Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for aliskiren (Tekturna): your body partly clears it using a liver enzyme called CYP3A4. In lab dish studies, the plumbagin in Ceylon Leadwort can slow that enzyme down. If that happened in people, less aliskiren would get broken down, so more could build up in your blood. That could nudge your blood pressure lower than intended and raise the odds of side effects like dizziness, lightheadedness, tiredness, or, less often, higher potassium levels.

The important part: this is only a theoretical concern based on test-tube work, not anything seen in actual patients, so I wouldn't panic. Still, since blood pressure swings matter, please check with your pharmacist or doctor before pairing the two rather than changing anything on your own.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.
