Aspirin, Codeine Phosphate, Phenobarbital falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

The piece of this combination pill that matters here is the codeine. Codeine is what we call a prodrug, meaning it does nothing on its own until your body converts it into morphine using a liver enzyme called CYP2D6. That morphine is what actually relieves your pain. Ceylon Leadwort contains plumbagin, which in lab dishes appears to block CYP2D6.

If that blocking happened in real people, less codeine would turn into morphine, so you might get weaker pain relief from this medicine. The good news is that this is only theoretical right now, based on test-tube studies and never reported in actual patients. It's worth a quick mention to your pharmacist or doctor, but no reason to panic.

ANTICOAGULANT/ANTIPLATELET DRUGS

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

The piece of this combination pill that matters here is the aspirin. Aspirin already thins your blood a bit by making platelets less sticky, which is exactly the same kind of effect that Ceylon Leadwort showed in lab and animal studies. Stacked together, the worry is more easy bruising, nosebleeds, bleeding gums, or stomach bleeding that is slower to stop.

I want to be honest about the evidence: this comes from test tube and animal research only, and it has not been confirmed in people, so it is more of a caution than a proven problem. Still, because bleeding can be serious, I would run it by your pharmacist or doctor before pairing them, especially if you take this aspirin product daily.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2B6 (CYP2B6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2B6. Drugs that are metabolized by CYP2B6 include ketamine (Ketalar), phenobarbital, orphenadrine (Norflex), secobarbital (Seconal), and dexamethasone (Decadron).

Here's the piece that matters in this combo pill: the phenobarbital. That barbiturate is partly cleared by a liver enzyme called CYP2B6. In test-tube studies, the plumbagin in Ceylon Leadwort slowed that same enzyme down. If that held true in people, your body might clear phenobarbital a little more slowly, letting it build up. That could mean extra drowsiness, dizziness, feeling foggy or unsteady, or slowed breathing, which adds to the sleepiness codeine already brings.

I want to be honest about how thin the evidence is. This has only been seen in the lab, never confirmed in real patients, so it's theoretical. Still, since phenobarbital is a touchy, narrow-margin drug, please check with your pharmacist or doctor before adding this supplement.
