Phenobarbital, Phenytoin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the situation with phenytoin (Dilantin) and Ceylon Leadwort. The plant compound plumbagin has been shown, in lab dish studies only, to block an enzyme called CYP2C9. That same enzyme is one of the main pathways your body uses to clear phenytoin. If it gets slowed down, phenytoin could in theory build up in your blood.

Why that matters: phenytoin has a narrow safety window, so even small jumps in its level can cause trouble like dizziness, unsteady walking, slurred speech, double vision, or drowsiness. To be clear, this has never actually been reported in people, so it's a theoretical concern. Still, since phenytoin is so sensitive, please check with your pharmacist or doctor before combining them.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2B6 (CYP2B6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2B6. Drugs that are metabolized by CYP2B6 include ketamine (Ketalar), phenobarbital, orphenadrine (Norflex), secobarbital (Seconal), and dexamethasone (Decadron).

Here's the situation with Ceylon Leadwort and your phenobarbital. The plant contains a compound called plumbagin, and in lab dish studies it slowed down an enzyme called CYP2B6, which is one of the pathways your body uses to break phenobarbital down. If that enzyme gets blocked, phenobarbital could theoretically build up in your system, which might mean more drowsiness, dizziness, unsteadiness, slowed breathing, or feeling extra sluggish.

I want to be honest with you, though: this has only been seen in test tubes, never in actual people. Phenobarbital is a medicine where small changes in level can matter, so I'd play it safe. Please check with your pharmacist or doctor before pairing them, rather than guessing.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
