Why this applies to Vincristine

Vincristine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the concern with this pairing. Vincristine is cleared from your body mainly by a liver enzyme called CYP3A4. A compound in Ceylon Leadwort, plumbagin, has been shown in lab dishes to slow that same enzyme down. If that happened in a real person, vincristine could build up to higher levels than intended, and that's a problem because this is a potent chemo drug with a narrow safety margin.

With more vincristine hanging around, you'd worry about worse nerve-related side effects like numbness, tingling, weakness, constipation, and low blood counts. The good news: this is only a theoretical interaction from test-tube data, never reported in actual patients. Still, since vincristine is so serious, please loop in your oncology team or pharmacist before adding this supplement.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Vincristine Sulfate Injection is approved to treat acute leukemia. It is also used as part of combination chemotherapy regimens for Hodgkin's disease, non-Hodgkin's malignant lymphomas, rhabdomyosarcoma (a soft-tissue cancer), neuroblastoma (a cancer of nerve tissue), and Wilms' tumor (a kidney cancer in children).

Because it works best alongside other cancer medicines, vincristine is almost always given as part of a multi-drug treatment plan rather than on its own.
