Ranolazine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the gist for ranolazine and chanca piedra. Ranolazine isn't a prodrug. It works as-is and your body relies heavily on an enzyme called CYP3A4 to clear it out. In lab (test tube) studies, chanca piedra appears to slow that same enzyme down. So in theory, pairing the two could let ranolazine build up higher than expected, which may mean more dizziness, nausea, constipation, or headache. The bigger concern with elevated ranolazine is QT prolongation, a change in heart rhythm.

I want to keep this in perspective: this is only a theoretical, lab-based finding and hasn't actually been seen in people. Still, ranolazine is sensitive to CYP3A4 changes, so before combining them, please run it by your pharmacist or doctor first.

What Is Ranolazine Used For?

Brand names: Ranexa, Aspruzyo

Ranolazine treats chronic angina — the recurring chest pain or pressure that happens when the heart isn't getting enough blood and oxygen. It is used when other angina medicines aren't enough on their own.

Both the extended-release tablet (sold as Ranexa and generic ranolazine) and the oral powder form (sold as Aspruzyo Sprinkle) are approved for this use. Ranolazine can be combined with other heart medicines such as beta-blockers, nitrates, calcium channel blockers, anti-platelet drugs, statins, ACE inhibitors, and angiotensin receptor blockers.
