Why this applies to Treprostinil

Treprostinil falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Here's the quick picture: treprostinil (Orenitram) is a prostacyclin-type medicine for pulmonary arterial hypertension. Besides relaxing blood vessels, it also makes platelets a little less sticky, so it can nudge your bleeding risk up on its own. Chanca piedra contains a compound that, in test-tube studies, slowed platelets from clumping together. Stacking these two could, in theory, make minor bleeding a bit more likely (think easier bruising, nosebleeds, or bleeding gums).

Now the reassuring part: this is only a theoretical concern based on lab work, not something seen in real patients. Still, since both can touch your platelets, I'd run it by your pharmacist or doctor before adding chanca piedra, and don't change your treprostinil on your own.

Theoretically, concomitant use of chanca piedra with antihypertensive drugs might have additive blood pressure lowering effects.
Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.

Here's the gist for Treprostinil (Orenitram). This medication widens your blood vessels to treat pulmonary arterial hypertension, and one of its built-in side effects is lowering your blood pressure. Chanca piedra has been studied for a mild blood-pressure-lowering effect of its own, so in theory the two together could nudge your blood pressure down a little more than expected.

I want to keep this in perspective. This concern is theoretical, drawn mostly from animal studies, and most people taking chanca piedra didn't actually see a drop in blood pressure. Still, if you notice extra dizziness, lightheadedness, flushing, or headache, check in with your pharmacist or doctor before changing anything. Don't stop either one on your own.

About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
