Chanca Piedra and Warfarin

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Chanca Piedra interacts with whole categories of medication. Warfarin falls into 3 of those categories, so it can interact with Chanca Piedra in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

1 of 3

Why this applies to Warfarin

Warfarin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea: warfarin already lowers your blood's ability to clot, and chanca piedra might nudge things a little further in that same direction. In a lab dish, one compound from this herb (methyl brevifolincarboxylate) slowed platelets from sticking together. Stack that on top of warfarin and, in theory, you could bleed a bit more easily.

I want to be straight with you though: this is a theoretical concern based on test-tube work, not something we've actually seen happen in people. Still, warfarin is a finicky drug where even small shifts matter, so I'd watch for things like easy bruising, nosebleeds, bleeding gums, or pink/dark urine. Please don't start chanca piedra without checking with your doctor or pharmacist first, since they may want to keep a closer eye on your INR.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Warfarin also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: in lab studies, chanca piedra seems to speed up an enzyme (CYP1A2) that helps break down warfarin. If that happened in your body, your warfarin could be cleared a little faster, which in theory might make it less effective and raise the small risk of a clot forming. So the concern here is a weaker blood thinner, not extra bleeding.

I want to be honest with you, though: this is only a theoretical, test-tube finding and hasn't been seen in real people. Still, warfarin is a drug where even small shifts matter, and your dose is guided by your INR blood test. Please don't start or stop anything on your own. Just loop in your pharmacist or the clinic that manages your INR before adding chanca piedra.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Warfarin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for warfarin. Lab studies suggest chanca piedra may slow down an enzyme called CYP3A4, which is one of the pathways your body uses to clear warfarin. If that enzyme gets blocked, warfarin could build up a bit and thin your blood more than intended. That matters because warfarin is a narrow-window drug, meaning even small shifts can tip you toward easy bruising, nosebleeds, or more serious bleeding.

The honest part: this is only a theoretical concern from test-tube research, not something reported in real patients, so I don't want to alarm you. Still, with warfarin I play it safe. Please loop in your pharmacist or doctor before combining them, and keep up with your regular INR checks.

About Chanca Piedra

What is Chanca Piedra?

Chanca Piedra
  • Kidney stones
  • Gallstones
  • Liver support
  • Urinary tract health
  • Digestive complaints

Chanca piedra is a tropical herb traditionally used as a 'stone breaker' for kidney and gallstones, and for liver and urinary health. Human evidence for these uses is limited and mostly small or preliminary, so it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a medical condition or take medications.

About Warfarin

What Is Warfarin Used For?

Brand name: Warfarin

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.

In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
