Here's the short version: Acenocoumarol (Sintrom) is a blood thinner, and like warfarin it has a narrow margin, meaning small shifts can tip you toward bleeding or clotting. The caffeine in cola nut may have a mild effect of its own that makes platelets a little less sticky. In theory, stacking that on top of your anticoagulant could nudge your bleeding risk up.

I want to be honest with you, though: this concern comes only from lab and animal data, and it has not actually been seen in people. The risk is considered unlikely. Still, watch for signs like easy bruising, nosebleeds, bleeding gums, or pink urine. If you enjoy cola nut regularly, just mention it to your pharmacist or doctor so they can keep an eye on your INR.
