Obeticholic Acid falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Cola Nut interacts with.
Cola nut contains caffeine. Caffeine is metabolized by CYP1A2,.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to notice with this pairing: the concern isn't really about your obeticholic acid (Ocaliva) becoming stronger or weaker. Instead, it runs the other direction. Cola nut is loaded with caffeine, and caffeine is cleared by a liver enzyme called CYP1A2. Obeticholic acid can mildly block that same enzyme, so in theory the caffeine in cola nut could hang around longer and build up.
If that happens, you might feel the usual caffeine overload: jitteriness, a racing heart, trouble sleeping, or feeling wired and anxious. This is only a theoretical concern based on how the drugs work, not something proven in studies, so try not to worry. Still, if you use cola nut regularly or notice these effects, it's worth a quick chat with your pharmacist or doctor before changing anything.

What Is Obeticholic Acid Used For?

Obeticholic acid is used alone or in combination with ursodiol (Actigall, Urso) to treat primary biliary cholangitis (PBC; a type of liver disease that destroys bile ducts, which allows bile to stay in the liver and cause damage) in people who cannot take ursodiol or in people who were not treated successfully with ursodiol alone. Obeticholic acid is in a class of medications called farnesoid X receptor agonists. It works by decreasing the production of bile in the liver and increasing the removal of bile from the liver.
