Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group Fever Bark interacts with.

Theoretically, fever bark might alter the effects of antihypertensive drugs.
Yohimbine, a constituent of fever bark, is an alpha-2 adrenoceptor antagonist and has been reported to increase blood pressure in clinical research. Conversely, reserpine can reduce both systolic and diastolic blood pressure. Theoretically, concomitant use of fever bark and antihypertensive drugs can interfere with blood pressure control.

Here's the gist with iloprost (Ventavis): it's an inhaled medicine that widens your blood vessels to ease the high pressure in your lungs and bring your blood pressure down a bit. Fever bark is tricky because it contains two opposing types of compounds. Yohimbine-like chemicals tend to raise blood pressure, while reserpine-like ones tend to lower it. So fever bark could pull your pressure in either direction and make iloprost's effect harder to predict.

In practice, that could mean dizziness, lightheadedness, headache, flushing, or feeling faint, especially when you stand up. Just so you know, this is a theoretical concern based on how these chemicals behave, not on solid human studies with iloprost itself. Still, fever bark is potent stuff, so please check with your pharmacist or doctor before combining them.

Theoretically, combining fever bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of fever bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of fever bark itself are unclear.

Here's the short version on pairing fever bark with iloprost (Ventavis). Besides relaxing the blood vessels in your lungs, iloprost also makes platelets a little less sticky, so it can mildly thin the blood. Fever bark contains a yohimbine-like compound that, in lab studies, also slowed platelet clumping. Put together, the theory is they could add up and make bleeding or bruising a bit more likely.

I want to be honest about how soft this is. The concern comes from test tube work, not from real people taking both, and the effect of fever bark itself is unclear. Still, if you notice easy bruising, nosebleeds, or bleeding gums, mention it. Please check with your doctor or pharmacist before adding fever bark.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.

Aurlumyn (iloprost injection) is FDA-approved to treat severe frostbite in adults. Specifically, it is used to reduce the risk of losing fingers or toes (digit amputation) after serious frostbite injury. The clinical evidence supporting this use comes from young, healthy adults who suffered frostbite at high altitudes.
