Acetaminophen, Dichlorophenazone, Isometheptane falls under STIMULANT DRUGS — the group Hordenine interacts with.

The piece of this combo medicine to focus on is isometheptene. It is a mild stimulant that works a bit like your body's own 'fight-or-flight' chemicals, gently tightening blood vessels to help ease a migraine. Hordenine is also marketed as a stimulant and may nudge norepinephrine in the same direction. Stacking two stimulant-type substances could, in theory, add up and raise your blood pressure or heart rate, possibly causing a pounding heart, jitteriness, or a headache that feels worse rather than better.
I'll be honest: this is a theoretical concern based on how these things are built, not on solid human studies, so don't panic. Still, since Isocom is already used for migraines and hordenine's safety isn't well established, I'd run this pairing by your pharmacist or doctor first, especially if you have any heart or blood pressure issues.
