This combo product has a lot going on, but the ingredient that connects to lentinan is the caffeine. Caffeine is cleared from your body mainly by an enzyme called CYP1A2. Lab studies hint that lentinan may speed that enzyme up, which could make caffeine break down a little faster. In plain terms, you might feel slightly less of caffeine's kick, less of that alertness or pick-me-up.
Here's the honest part: this is only seen in test-tube and animal research, never confirmed in people, so I wouldn't lose sleep over it. The other pieces (acetaminophen, hydrocodone, chlorpheniramine, phenylephrine) aren't really affected by this pathway. If you take lentinan regularly, just mention it to your pharmacist so they can keep an eye on things.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the wrinkle with this combo: that cold-and-cough product contains hydrocodone, which is a prodrug. Your liver uses an enzyme called CYP2D6 to convert hydrocodone into hydromorphone, the form that actually relieves pain and quiets a cough. Lab and animal studies hint that lentinan may speed up CYP2D6. If that held true in people, it could mean a little more of the active painkiller is made, theoretically nudging up effects like drowsiness, dizziness, constipation, or slowed breathing.
I want to be honest, though: this is only test-tube and animal data, and it has never been seen in real patients. So it's worth a quick word with your pharmacist or doctor before combining them, but it is not cause for alarm.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

This combination product mixes a few different ingredients, but the one worth watching here is hydrocodone, the opioid for pain and cough. Hydrocodone is actually a prodrug in part: your liver's CYP3A4 and CYP2D6 enzymes help process it. Lab studies hint that lentinan might slightly speed up CYP3A4. In theory, faster breakdown could shift how much active drug your body makes, which might mean a bit less pain or cough relief, or simply a different feel from your usual dose.
