Acetaminophen, Chlorpheniramine, Phenylephrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Lentinan interacts with.

Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Here's the short version: this combo cold product (the acetaminophen for pain and fever, chlorpheniramine for sniffles, and phenylephrine as a decongestant) isn't a great match for the CYP1A2 concern with lentinan. Lab work hints that lentinan may speed up the CYP1A2 enzyme, which could clear certain drugs a bit faster and make them work less well. Acetaminophen does get touched by CYP1A2 to a small degree, but its main breakdown pathways are different, so this isn't a big worry.

Honestly, the bigger point is that this is only test-tube and animal evidence, never shown in real people. For occasional cold-medicine use, I wouldn't lose sleep over it. Still, run it by your pharmacist if you take lentinan regularly.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist with this combo cold and allergy product (acetaminophen for pain and fever, chlorpheniramine for allergy symptoms, and phenylephrine as a decongestant). Lab studies hint that lentinan might speed up a liver enzyme called CYP3A4. If that happened in the body, drugs cleared by that enzyme could be broken down a little faster, which in theory might make them work a touch less well.

Honestly though, this is very weak, theory-only evidence from test tubes and animals, not people. None of these three ingredients lean heavily on CYP3A4, so I wouldn't expect a meaningful change in your stuffy nose, sneezing, or fever relief. Still, if you plan to use lentinan regularly, run it by your pharmacist or doctor first.
