Why this applies to Hydrochlorothiazide, Metoprolol

Hydrochlorothiazide, Metoprolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Your combo pill pairs metoprolol (a beta-blocker) with hydrochlorothiazide (a water pill). The metoprolol half is the piece that matters here. It's cleared by a liver enzyme called CYP2D6, and lab studies hint that lentinan might speed that enzyme up. If that happened in real life, your body could clear metoprolol a little faster, meaning slightly weaker effect, so your heart rate or blood pressure might creep up. The hydrochlorothiazide part isn't really affected.

Here's the reassuring part: this is only seen in test-tube and animal research, and it has never been reported in actual patients. So I wouldn't lose sleep over it. Still, if you decide to try lentinan, keep an eye on your blood pressure and let your pharmacist or doctor know so they can check in.

Your combo pill pairs metoprolol (a beta-blocker that slows heart rate and lowers blood pressure) with hydrochlorothiazide (a water pill). The thing to know here is about the metoprolol half. Your liver clears metoprolol using an enzyme called CYP2D6, and in animal studies lentinan seemed to speed up that enzyme. If that holds true in people, your body could clear metoprolol a little faster, which might make it work less well.

In real life that could look like a slightly higher heart rate or blood pressure than usual. I want to be honest though: this comes only from animal research, so it's far from proven, and oral lentinan products vary a lot. Before adding it, check in with your pharmacist or doctor, and don't change your blood pressure medicine on your own.
