Why this applies to Ibrutinib

Ibrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for Imbruvica (ibrutinib). Your body relies heavily on an enzyme called CYP3A4 to clear this medication. Lab studies hint that lentinan might rev up that enzyme, which could mean ibrutinib gets broken down faster and your blood levels drift lower than intended. Since ibrutinib is the active drug (not a prodrug that needs switching on), lower levels would likely mean less cancer-fighting benefit, not more side effects.

I want to be honest about the evidence though: this is based only on test-tube and animal research, and it hasn't actually been seen in people. Still, with a cancer treatment this important, don't add lentinan without first checking with your oncologist or pharmacist.

What Is Ibrutinib Used For?

Brand name: Imbruvica

Imbruvica (ibrutinib) is used in adults to treat chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL), including cases with a specific genetic change called 17p deletion. It is also used in adults to treat Waldenström's macroglobulinemia (WM), a rare type of slow-growing blood cancer.

Imbruvica is also approved for adults and children aged 1 year and older with chronic graft-versus-host disease (cGVHD) — a serious immune complication that can occur after a stem cell or bone marrow transplant — when at least one prior treatment has not worked.
