Why this applies to Sirolimus (albumin bound)

Sirolimus (albumin bound) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the basic idea with Fyarro (the albumin-bound form of sirolimus). Your liver uses an enzyme called CYP3A4 to break this medicine down. Lab studies hint that lentinan might speed up that enzyme. If that happened in real life, your body could clear sirolimus a little faster, which might lower its levels and, in theory, make it less effective against the tumor it's treating.

I want to keep this in perspective, though. This concern comes only from test-tube and animal research, and it has not actually been seen in people taking lentinan. Sirolimus is a careful, narrow-margin cancer drug, so even small changes matter. Please don't start or stop anything on your own. Check in with your oncology team or pharmacist before adding lentinan.
