Why this applies to Tramadol

Tramadol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the wrinkle with tramadol: it's actually a prodrug. Your liver uses an enzyme called CYP2D6 to turn it into its more powerful pain-fighting form (called O-desmethyltramadol). So this enzyme isn't just clearing the drug, it's switching it on.

Lab research hints that lentinan might speed up CYP2D6. If that happened in people, it could mean tramadol gets converted faster, which might give you stronger pain relief but also more opioid side effects like drowsiness, nausea, dizziness, or constipation. That said, this is only test-tube and animal data, and it has never been reported in actual patients, so I wouldn't lose sleep over it. Still, mention any lentinan use to your pharmacist or doctor before combining the two.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the wrinkle with tramadol: it's actually a prodrug. Your liver has to turn it into its active form before it does much for pain, and the enzyme CYP3A4 is one of the players involved in processing it. Lab and animal research hints that lentinan might rev up CYP3A4 a bit.

If that held true in people, tramadol could clear out a little faster, which in theory might leave you with weaker pain relief. But honestly, this is only a test-tube and animal finding, with nothing reported in real patients, so I wouldn't lose sleep over it. Still, if you start lentinan and notice your pain isn't as well controlled, mention it to your pharmacist or doctor before changing anything.
