Why this applies to Vinblastine

Vinblastine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the idea with shiitake-derived lentinan and your vinblastine. Vinblastine is cleared from your body mostly by a liver enzyme called CYP3A4. Lab and animal studies hint that lentinan might speed that enzyme up. If that happened in a person, your body could break down vinblastine a little faster, which in theory might lower its levels and make the chemo work less well.

I want to be honest about the strength here: this is only test-tube and animal data, and it has never actually been seen in humans, so don't panic. Still, with a cancer drug, even small changes in level matter. Please don't start or stop lentinan on your own. Run it by your oncology team or pharmacist first so they can keep your treatment on track.

What Is Vinblastine Used For?

Brand name: Velban

Vinblastine Sulfate Injection is used in the treatment of a range of cancers. Conditions that tend to respond well include generalized Hodgkin's disease, lymphocytic lymphoma, histiocytic lymphoma, advanced mycosis fungoides (a type of skin lymphoma), advanced testicular cancer, Kaposi's sarcoma, and Letterer-Siwe disease (a rare immune condition also called histiocytosis X).

It may also be used for cancers that respond less reliably, such as choriocarcinoma resistant to other chemotherapy drugs, and breast cancer that has not responded to hormonal therapy. Vinblastine is often given as part of a combination chemotherapy regimen alongside other cancer medicines to improve response rates.
