Why this applies to Mephenytoin

Mephenytoin falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.

Theoretically, concomitant use with hepatotoxic drugs might increase the risk of adverse hepatotoxic effects.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.

Here's the simple version. This isn't about one drug speeding up or slowing down the other. Both Ostarine and Mephenytoin can be hard on your liver on their own, so the worry is that stacking them may add up and raise your chance of liver trouble. Mephenytoin (an older seizure medicine) already carries a known risk of liver injury, and Ostarine, an unapproved SARM, has been tied to raised liver enzymes and even reports of drug-induced liver damage.

To be fair, this concern is theoretical, based on how each affects the liver rather than studies of the two together. Still, watch for yellowing skin or eyes, dark urine, belly pain, nausea, or unusual tiredness, and check with your doctor or pharmacist before combining them.

Theoretically, concomitant use of ostarine with CYP3A4 inducers could decrease the clinical effects of ostarine.
Ostarine is partially metabolized by CYP3A4. Clinical research shows that taking rifampin, a potent inducer of CYP3A4, reduces the maximum plasma concentration of ostarine by 23% and the area under the curve by 43%.

Here's the key thing to understand: in this pairing, Mephenytoin is the one doing the affecting, and Ostarine is the one being affected, not the other way around. Mephenytoin (an older seizure medicine) can speed up a liver enzyme called CYP3A4, which is one of the enzymes that breaks down Ostarine. So if you took both, your body would likely clear the Ostarine faster and you'd have less of it around. In studies with a strong enzyme booster, Ostarine levels dropped by roughly a quarter to nearly half.

Honestly though, my bigger concern is Ostarine itself. It's an unapproved SARM, it's banned in sports, and it has been tied to liver injury. I wouldn't recommend using it at all. Please talk with your pharmacist or doctor before combining anything with your seizure medicine.

Mephenytoin also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, ostarine might increase levels of drugs metabolized by CYP2C9, although clinical research suggests this is not clinically relevant.
Although in vitro research suggests that ostarine inhibits CYP2C9, more robust clinical research shows that ostarine does not significantly affect CYP2C9.

Here's the short version on combining Ostarine with Mephenytoin (Mesantoin), an older seizure medicine. Mephenytoin is cleared in part by a liver enzyme called CYP2C9. Early lab (test tube) work hinted that Ostarine might slow that enzyme down, which in theory could let mephenytoin build up and cause more side effects like drowsiness, dizziness, unsteadiness, or confusion. The good news is that better human studies suggest Ostarine does not meaningfully affect CYP2C9, so this concern stays mostly theoretical.

That said, mephenytoin is a touchy drug with a narrow safe range, so I never take chances. Bigger picture, Ostarine itself is an unapproved SARM linked to liver harm, which I'd want you to discuss with your doctor or pharmacist before using anything alongside your seizure medicine.
