Here's the situation with red clover and your eczema medicine, Cibinqo (abrocitinib). Your body partly clears this drug using a liver enzyme called CYP2C19. In lab dish studies, red clover's plant estrogens appear to weakly slow that same enzyme down. If that happened in real life, abrocitinib could clear a bit more slowly and build up slightly, which in theory might nudge up its effects and side effects (things like nausea, headache, acne, or a greater chance of infections since this drug calms the immune system).

That said, I want to keep this in perspective. This is only a theoretical, test-tube finding and it has never actually been reported in people. The effect looks weak and unlikely to cause trouble. Still, it's worth a quick mention to your pharmacist or doctor before you start red clover.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the short version: this one is mostly theoretical and not something I'd lose sleep over. Abrocitinib (Cibinqo) is partly cleared by a liver enzyme called CYP2C9. In a test-tube setting, red clover's isoflavones looked like they might slow that enzyme down a little, which in theory could let abrocitinib build up and slightly raise the odds of side effects like nausea, headache, or a higher infection risk.

But here's the reassuring part: an actual human study using standardized red clover did not change how the body handled a CYP2C9 drug. So real-world impact looks unlikely. Abrocitinib isn't a prodrug, so it doesn't flip direction. Still, since this medicine affects your immune system, it's worth a quick word with your pharmacist or doctor before adding red clover.

Although some laboratory research suggests that red clover may have anticoagulant and antiplatelet activity, clinical research has not shown this effect.
In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.

Here's the honest picture for abrocitinib (Cibinqo). The interaction we have on file ties red clover to blood thinners and anti-platelet medicines because lab studies hinted it might thin the blood a little. But here's the catch: abrocitinib isn't a true anticoagulant. It's a JAK inhibitor that calms an overactive immune system to help your eczema, and one of its known side effects can be a drop in platelets, which can raise bruising or bleeding risk.

So the concern would only be a small, theoretical added bleeding tendency if both nudged in the same direction. In reality, good human studies found red clover did not change clotting at all, so this is rated minor and unlikely. I still like a quick chat with your pharmacist before adding it.

What Is Abrocitinib Used For?

Brand name: Cibinqo

Cibinqo (abrocitinib) is used to treat moderate-to-severe atopic dermatitis (eczema) in adults and children 12 years of age and older. It is specifically for people whose eczema has not been adequately controlled with other systemic (whole-body) treatments, including biologics, or when those treatments are not a safe option.

Cibinqo is not recommended to be used at the same time as other JAK inhibitors, biologic immunomodulators, or other immunosuppressants (medicines that reduce immune system activity).
