Acetaminophen, Butalbital, Caffeine, Codeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
This combo pill has a few moving parts, but the piece red clover might touch is the caffeine. In a test tube, red clover can slow down CYP1A2, the liver enzyme that clears caffeine. In theory, that could let caffeine linger a bit longer and add to jitteriness, a racing heart, or trouble sleeping.
Here's the reassuring part: this is only lab-dish evidence, and when researchers actually gave people red clover isoflavones for two weeks, caffeine levels didn't budge. So a real-world effect is unlikely. The codeine, acetaminophen, and butalbital aren't cleared by this pathway, so they shouldn't be affected. Still, if you notice extra restlessness, mention it to your pharmacist or doctor before changing anything.

However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist with this combo pill (Fioricet with Codeine). In a lab dish, red clover's isoflavones looked like they might slow down CYP3A4, one of the liver enzymes that helps break down the butalbital portion of this medicine. In theory, slower breakdown could let levels creep up, which might mean more drowsiness, dizziness, or that foggy, sluggish feeling. But here's the reassuring part: when researchers actually tested red clover in people using a similar CYP3A4 drug, they saw no real change. So this is mostly a theoretical concern, not something proven in real life.
I wouldn't lose sleep over it, but since this pill already causes sedation, it's worth a quick mention to your pharmacist or doctor, especially with supplement-strength doses rather than a casual cup of tea.

CAFFEINE

Red clover contains genistein. Taking genistein 1 gram daily for 14 days seems to inhibit caffeine clearance and metabolism in healthy females. However, this effect does not seem to occur with the lower amounts of genistein found in red clover. A clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine.

The only piece of your Fioricet with Codeine combo that interacts here is the caffeine. In theory, the genistein found in red clover could slow how fast your body clears caffeine, which would let it linger a little longer. If that happened, you might notice more of caffeine's usual effects, like jitteriness, a racing heartbeat, or trouble sleeping.
Here's the reassuring part: this concern only showed up at very high, concentrated genistein doses (about 1 gram a day). The much smaller amount of isoflavones in normal red clover supplements did not change caffeine levels in studies. So in real life this is unlikely to cause you any trouble. Still, it's always smart to run any new supplement past your pharmacist or doctor first.
