Why this applies to Asenapine

Asenapine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the gist for asenapine (Saphris, Secuado). Your body clears this medicine partly through a liver enzyme called CYP1A2. In a test tube, red clover seems to slow that enzyme down a little, which in theory could let asenapine build up and bring on more of its usual effects, things like drowsiness, dizziness, low blood pressure when you stand, or restlessness.

But I want to keep this in perspective. This is theoretical, based on lab work, not real people taking the two together. When researchers actually tested red clover in volunteers using a CYP1A2 marker, they saw no meaningful change. So a real-world problem is unlikely. Still, it's worth a quick mention to your pharmacist or doctor before adding red clover.

What Is Asenapine Used For?

Brand names: Saphris, Secuado

Asenapine treats schizophrenia in adults and bipolar I disorder (manic or mixed episodes) in adults and in children ages 10 to 17. It can be used alone or alongside lithium or valproate in adults with bipolar I disorder.

The sublingual tablet forms (Saphris and the generic asenapine sublingual tablet) are approved for both schizophrenia and bipolar I disorder. Secuado, the skin patch, is approved only for schizophrenia in adults.
